Women’s health in Bakersfield — inside primary care.
LAC Women’s Health is a part of LAC Medical, not a separate clinic. Menopause and hormone therapy, the women’s wellness exam, and the bone, heart, and mood care that goes with them are handled in the same Bakersfield office that manages your blood pressure, your thyroid, and your annual visit.
Primary care is what we do. Women’s health is part of how we do it — same practice, same chart, same front desk, no separate specialty referral and no separate clinic fee. Spanish is spoken here every day we are open. Accepting new patients.
What “inside primary care” actually means
Most women’s health care in Bakersfield is organized as a referral: you raise something at a primary care visit, you get sent somewhere else, and you wait. That is the step we removed.
At LAC Medical, women’s health is a part of the primary care practice. You are not being handed off to a different office with a different chart, a different front desk, and a different wait list. It is the same practice, the same medical record, and the same people who already know what changed since your last visit.
Practically, that means four things.
- No separate referral. If you are already an LAC patient, bring it up at your next visit. If you are new, ask for it when you schedule. There is no gatekeeping step in between.
- One chart, one team. Your menopause plan, your blood pressure, your cholesterol, your thyroid, and your medication list all live in one record, reviewed by the same providers. Nothing gets managed in isolation from everything else.
- Billed as primary care. This is a primary care office visit at standard primary care rates. No separate women’s health clinic fee, no membership, no concierge add-on, and nothing sold to you at the visit.
- Bilingual, every day. A Spanish-speaking provider and Spanish-speaking front-desk staff are here every day the office is open, and this page and our menopause pages exist in Spanish as well as English.
Some things do belong outside primary care: abnormal bleeding, including bleeding after menopause; fibroids that need imaging; complex bone disease; and hormone therapy decisions genuinely complicated by other conditions. We have the referral network, and we manage the handoff so you are not left to navigate it alone.
Primary care is the practice. Women’s health is one wing of it, not a second business — see everything primary care covers.
What we cover
Everything below is delivered as part of a primary care visit at 5300 Lennox Avenue in Bakersfield. Where something is outside primary-care scope, we say so and we manage the referral rather than leaving you to chase it.

Menopause and hormone therapy
Perimenopause and postmenopause symptom evaluation and management, and FDA-approved hormone therapy when it is the right tool — transdermal estrogen, oral progestogen, topical vaginal estrogen. Lowest effective dose to start, follow-up scheduled, and stopping rules built in from the beginning. Non-hormonal FDA-approved options for patients who cannot or prefer not to use hormones. We do not prescribe compounded pellets, troches, or creams.

Vaginal and urogenital care
Local low-dose estrogen, moisturizers, and pelvic-floor referrals when appropriate. Local therapy has a different risk profile than systemic therapy, and low-dose vaginal estrogen remains appropriate at any age, including past 60 and more than ten years after your last period.

Bone, heart and mood
Bone density screening on the schedule that fits your risk. Cardiovascular risk review at every annual visit. Depression and anxiety screening, with treatment when it is indicated. This is where the women’s health visit and the rest of your primary care meet.

Women’s wellness exam
The women’s wellness exam is delivered as part of primary care here, in the same office and on the same chart as the rest of your care. If you are on Medicare, Medicare’s free Annual Wellness Visit is a reasonable place to open the conversation. Ask the front desk what to expect when you schedule, or bring it up at your next visit.

The rest of your care, same office
Diabetes, high blood pressure, high cholesterol, thyroid, and respiratory conditions are managed by the same team. Adult immunizations, in-office lab draws, EKG, and spirometry are here too. Cancer screening — colonoscopy, mammogram, and lung cancer screening — is ordered and coordinated from this office; the test itself is done at the imaging center or with the appropriate specialist. LAC Medical is an adult practice — we do not see children.
Where menopause fits
Menopause is the front door of this wing, and it has its own page because there is a lot to say about it.
Perimenopause and postmenopause symptoms — hot flashes and night sweats, sleep disruption, mood changes, brain fog, joint pain, urogenital symptoms — are evaluated and treated here. When hormone therapy is the right tool, we prescribe FDA-approved products: transdermal estrogen, oral progestogen, topical vaginal estrogen. We do not prescribe compounded pellets, troches, or creams. When hormone therapy is not the right tool, there are FDA-approved non-hormonal options and we start one the same day.
The visit itself is structured: a validated symptom scale, a history and risk review, labs only when they will change the plan, a written plan before you leave, and a three-month follow-up for anything new. In November 2025 the FDA removed the boxed warning on systemic estrogen products, concluding it had overstated the risks for menopause symptoms — that change is why more primary care offices are willing to have this conversation, and it is the labeling we follow.

Peyman Sarrafian, MD
Family Medicine · leads LAC Women’s Health
Dr. Sarrafian is a family practice physician whose clinical focus is menopause and hormone therapy. He completed his family medicine residency at Kern Medical Center in 2010 and has practiced in Bakersfield for more than fifteen years. In July 2026 he presented “Modern Medical Menopause Management,” a provider education evening for Bakersfield-area clinicians. He treats menopause the way the rest of this practice treats long-term conditions — as ordinary primary care with structured follow-up, not as a specialty consult.

Rocio Argueta, NP
Nurse Practitioner · continuity anchor
Most patients at LAC see Rocio Argueta, NP. She runs the panel and is the continuity anchor across visits: the same provider, visit after visit, who knows what changed since the last one. Patients choose LAC for her, and that is by design.
That is the whole clinical team. It is a small practice on purpose, and it is the reason continuity is something we can actually promise.
Insurance and cost
Women’s health visits at LAC are billed as primary care office visits at standard primary care rates. There is no separate clinic fee for this line of care.
We accept Medicare Part B, Qualcare HMO, DHMN HMO, and the major commercial plans. We do not accept Medi-Cal or Kern Family Health Care — we say that up front so no one drives across town to find out at the front desk.
If you are on Medicare, Medicare’s free Annual Wellness Visit is a reasonable place to start the conversation, with a follow-up visit billed normally from there. Medicare Part D covers FDA-approved hormone therapy prescriptions according to your plan’s formulary, and we prefer generics where they exist. There are no proprietary supplements, no panel testing kits, and no pellet implants sold here.

Common questions
Do I need a referral to be seen for women’s health at LAC?
No. Women’s health is part of primary care here. If you are already an LAC patient, raise it at your next visit. If you are new, request an appointment and say what you would like to discuss. There is no separate specialty referral and no separate wait list.
Can a family practice office really handle menopause?
Yes, for most patients. Major guideline bodies, including the American College of Obstetricians and Gynecologists and The Menopause Society, support menopause care in primary care for the majority of patients. We refer out when something falls outside that scope — abnormal bleeding, fibroids that need imaging, complex bone disease, or a hormone therapy decision genuinely complicated by other conditions.
Do you prescribe pellets or compounded hormones?
No. We prescribe FDA-approved hormone therapy products only. Bioidentical molecules such as estradiol and micronized progesterone are used in their FDA-approved forms. Compounded pellets, troches, and creams are not approved by the FDA and their dosing is not standardized, so we do not prescribe them.
Is this billed through insurance, or is it a cash program?
It is billed through insurance as a primary care office visit. We accept Medicare Part B, Qualcare HMO, DHMN HMO, and the major commercial plans. We do not accept Medi-Cal or Kern Family Health Care. There is no membership, no concierge tier, and no cash add-ons at the visit.
