Independent primary care for adults

Primary care works when one practice knows your history, tracks your numbers, and picks up the phone. That is a relationship measured in years, and we are built for it. Our staff stays. Our schedule respects your time. Our recommendations come from your clinical team, not from a corporate playbook. When you need a specialist, we have the network. When you don’t, we tell you so.

Our office is at 5300 Lennox Avenue, Suite 105, and we are open Monday through Friday, 8:00 AM to 5:00 PM. Request an appointment using the form at the bottom of this page or call (661) 735-1710. We answer online requests within one business day, and most new patients are seen within the same week of getting in touch. Who we see

LAC Medical is an adult practice. Our declared specialties are family medicine, internal medicine, and geriatric medicine, and our panel runs from working-age adults through Medicare patients. We do not see children, so we are not the place for whole-family care.

People usually come to us as one of the following:

  • Adults who want a regular doctor, whether or not something is wrong right now.
  • Adults living with one or more long-term conditions — diabetes, high blood pressure, high cholesterol, or a thyroid disorder.
  • Patients new to Medicare who want the Welcome to Medicare Visit (IPPE) done correctly in their first year of Part B.
  • Patients on Medicare Part B who want the Annual Wellness Visit on the calendar every year.
  • Adults leaving a large system who want a practice small enough to know them.
  • Patients coming home from the hospital who need follow-up in the days right after discharge.

Long-term condition care

Ongoing conditions are the core of what we do. Managing one well is not a single appointment — it is routine bloodwork, medication that gets reviewed instead of refilled on autopilot, attention to complications before they start, and a follow-up already scheduled before you leave.

We manage diabetes, high blood pressure, high cholesterol, and thyroid disorders as continuous care. Three areas have their own pages, because the detail matters:

  • Diabetes care

    A1c tracking, medication adjustment, and the screenings that protect your eyes, kidneys, feet, and heart — plus the referrals when a specialist should be involved.

  • Blood pressure and heart health

    Blood pressure that is actually followed between visits, cholesterol managed alongside it, cardiovascular risk screening, and an in-office EKG when your symptoms call for one.

  • Asthma and respiratory care

    Breathing problems get measured, not guessed at: we do spirometry — lung function testing — in the office, and we manage ongoing respiratory conditions the same way we manage any other chronic condition.

High cholesterol and thyroid disorders are managed here in the same rhythm — labs, a dose adjusted to your results, and a plain explanation of what the numbers mean for you.

For patients on Medicare with two or more ongoing conditions, we also offer Chronic Care Management (CCM), a Medicare service that pays for the coordination that happens between your visits: tracking results, adjusting medications, and staying in contact with the specialists involved in your care.

Preventive care and Medicare wellness visits

Preventive care is the part of primary care that keeps problems small. At LAC it is built around what your plan already covers and what actually changes your risk.

For Medicare patients, the anchor is Medicare’s free Annual Wellness Visit — no copay, no deductible. It is not a physical exam. It is a yearly planning visit: we go through your health history, review every medication you take, screen for the risks that matter at your age, and leave you with a written schedule of the screenings you are due for. Medicare covers it once every twelve months.

What we handle under preventive care:

  • Medicare’s free Annual Wellness Visit (AWV) — no copay, no deductible.
  • The Welcome to Medicare Visit (IPPE), for patients in their first twelve months of Part B.
  • Adult immunizations — flu, shingles, pneumonia, Tdap, and COVID-19.
  • Cardiovascular risk screening.
  • Depression and anxiety screening, with treatment when it is indicated.
  • Cancer screening referrals — colonoscopy, mammogram, and lung screening. We order and coordinate these; the screening itself is performed by the imaging or specialty center we send you to.
  • Advance Care Planning, if you want your wishes documented before anyone needs them.

Sick visits and same-week appointments

Not every visit is planned. Colds, infections, a cough that will not clear, a new symptom you would rather not sit on over the weekend — those are primary care visits, and they belong with the practice that already has your chart.

Established patients can usually get a same-day visit for an urgent concern. New patients are usually seen within the same week of getting in touch. Call (661) 735-1710 in the morning if you need to be seen that day; the sooner we hear from you, the more the schedule can absorb.

If you have just been discharged from a hospital, tell us when you call. Transitional Care Management (TCM) is a covered Medicare service for the weeks after a hospital stay, when medication changes and missed follow-ups cause most readmissions, and it works better the sooner the visit happens.

Testing done in the office

Three things we do not send you across town for:

  • Lab draws. Routine blood work is drawn here, at your visit, by our staff.
  • EKG. An electrocardiogram, done in the office when your heart symptoms or your cardiovascular risk call for one.
  • Spirometry. Lung function testing, done in the office, so a breathing problem gets a measurement instead of an assumption.

When something is outside what we do here — imaging, a procedure, a specialist opinion — we order it and coordinate the handoff so you are not left to chase it down yourself.

Women’s health, inside primary care

LAC Women’s Health is a program inside this Bakersfield primary care practice, led by Dr. Peyman Sarrafian, MD. It covers menopause care, FDA-approved hormone therapy, and the women’s wellness exam — delivered as part of primary care, with no separate specialty referral and no separate clinic fee. There are two providers at LAC Medical, and you will get to know both of them.

    Rocio Argueta, NP

    Most patients at LAC see Rocio Argueta, NP. She runs the panel and is the continuity anchor across visits — the same provider, visit after visit, who knows what changed since the last one. Patients choose LAC for her, and that is by design.

  • Dr. Peyman Sarrafian — family practice physician at LAC Medical

    Peyman Sarrafian, MD

    Dr. Peyman Sarrafian is our family practice physician. He handles complex care and leads LAC Women’s Health.

That is the whole clinical team. It is a small practice on purpose, and it is the reason continuity is something we can actually promise.

What a first visit looks like

Becoming a patient takes three steps.

  1. Get in touch.

    Request an appointment with the form at the bottom of this page, call (661) 735-1710, or email contact@lacmedicine.com. The form asks for three things — your name, your phone number, and the best time to call you. Please keep medical details out of email and the form; save those for the visit. Our front desk calls you back, confirms your insurance and the reason for the visit, and books the soonest fit.

  2. Come in for a full new-patient visit.

    Bring your insurance card, a photo ID, and a current list of your medications. That is all. We take care of the paperwork, and if you would rather fill the forms out in advance, they are on our patient resources page in English and Spanish. Our front desk will help you complete them at check-in at no charge, and Spanish-speaking staff can read them to you.

  3. Leave with a plan.

    Labs ordered, medications reviewed, screening referrals if you need them, and a follow-up scheduled before you walk out. At the first visit you will also sign an acknowledgment of our Notice of Privacy Practices; you can read it in advance in our HIPAA Notice of Privacy Practices.

After that, the patient portal is yours. Through healow at health.healow.com/LACMEDICALINC you can request an appointment, pre-register, view your records and lab results, and pay a bill.

Insurance and cost

We are direct about coverage, because finding out at the front desk is the wrong time to find out.

If you don’t see your plan, call (661) 735-1710 — most major commercial plans are accepted. We take insurance, credit cards, and debit cards.

Note: We do not accept Medi-Cal or Kern Family Health Care. If that is your coverage, we would rather tell you now than waste your appointment.

What you get here is comprehensive primary care for adults — not a menu of upsells. There is no membership, no concierge add-on, and nothing sold to you at the visit. Care is insurance-covered when applicable, and we are transparent when it isn’t.

For referring providers

We accept primary-care referrals from specialists, hospital discharges, and community providers across Bakersfield and Kern County. Fax referrals to (661) 888-4841. Hospital-discharge referrals are prioritized for post-discharge follow-up under Transitional Care Management, so send them as soon as the discharge date is set.

The office is wheelchair accessible — parking, entry, exam rooms, and restrooms. We provide ASL interpreters, large print, audio recordings, and staff reading assistance at no charge, and free telephone interpreters for languages other than English and Spanish.
